The US does not have a healthcare system, but we do have a fantastic sickcare system if you want to be unwell. We really suck at wellness and reversing lifestyle-induced diseases. We certainly aren't preventing them. And it's even more sad than this because the truth is, we don't really want to do better (although we tell ourselves and each other that we do). When I was in medical school, we were taught that lifestyle change was always the first tool for the treatment of disease. I rarely saw this practiced in residency and in the decade following, I can't remember the last time a doctor told me to change my lifestyle before prescribing a medication. If this is happening to me as a physician when I'm a patient, I know it is happening everywhere else as well. But this is not new. The government is not going to change this. Neither is insurance. Or hospitals and healthcare systems. These Goliaths of "healthcare" have every motivation and bias to NOT change. Don't believe me? How much were the executives at these companies paid last year? And yet insurance premiums continue to rise, claims are denied, medications are withheld, diagnostic tests are delayed and patient responsibility out-of-pocket costs are bankrupting people. I'm not even going to pretend that we will see change in the foreseeable future. So what should we do? Each of us, as individuals, must break free from the paradigm of managing chronic diseases with medication. We must adopt a mindset centered on preventing disease, reversing disease when present and sustaining healthy changes over time. We must change our habits. This means we also must change the thoughts leading to these habits. We must be intentional. I believe we are the David in this story and it's time for each of us to take individual action in our own health. What do you think we should do? Share your insights, stories and ideas below. Question sent to Ask Julian: I have a family member living in a nursing home. His nursing home costs are paid by Medical Assistance (Medicaid). He owns a home in his name only.
