How it works

Get great eye care with low copays and flexible coverage. You can use the plan at any Vision Service Plan (VSP) network provider nationwide, including most Walmart Vision Centers and Sam’s Club Optical. Before you get service, make sure ​your provider​ is ​in the​ VSP network.

  • Eye exams:$4 copay, once every calendar year.
  • Frames:$130 frame allowance, once every calendar year.
  • Lenses:$4 materials copay when you buy frames and lenses together.
  • Progressive lenses:additional $55 copay.
  • Contact lens evaluation:up to $60 copay.
  • Contact lenses:$130 allowance, once every calendar year instead of glasses.

ID cards
  • If you’re enrolled in both a Walmart medical plan and the vision plan, you’ll find VSP contact information on your plan ID card.
  • If you’re enrolled in an HMO or have no medical coverage through Walmart, you’ll get a separate VSP ID card mailed to your home.
Using the plan
  • You can get care at a Walmart Vision Center, Sam’s Club Optical, or VSP network provider. NOTE: Confirm that the provider at Walmart Vision Center or Sam’s Club Optical is a VSP network provider.
  • Let them know you’re a VSP member, and present your medical or vision plan ID card if you have one.
  • Pay your copay and any other required charges during your visit. Your provider and the plan will take care of the rest.
  • If for some reason your benefit isn’t handled automatically, you can file a claim through theplan’s website.
Enrolled in an HMO?

Many HMOs offer their own vision coverage. If yours does, compare the cost and benefits before you decide whether to also enroll in vision coverage​.
